Service Support Officer Care Navigation - what's involved in the role?
The role of the Service Support Officer (SSO) Care Navigation works within the Mission and Mission in Action of HammondCare to ensure that all home care services are tailored, personal, and delivered to the highest standards. The broad range of administration activities delivered by our SSOs, for our teams, potential clients, clients, representatives, and contractors are provided both locally and centrally. This role is key in supporting various areas and regions with strong administrative delivery. The SSO may support one or more of the following areas across HammondCare at Home (HCAH), which operate both locally and/or centrally:
* Care Navigation
The administrative activities in HammondCare at Home are essential to daily operations and planning, supporting care teams, client care, potential clients, and contractors who assist in providing and planning care and services.
Key Focus Areas:
* Provide reliable and consistent administrative support for colleagues, potential clients, clients, their representatives, and contractors across our Care Navigation Service, Partner Services, and Areas/Regions.
* Work closely with colleagues and teams to ensure best practices and consistent ways of working to meet regulatory, service, and care requirements.
* Support and promote positive communication, collaboration, and engagement within the team and with internal and external stakeholders.
* Focus on stream activity and support other streams as needed for seamless administration and support across HCAH.

* Previous experience in an administrative role or similar.
* Effective communication and interpersonal skills.
* Experience in administrative, customer service, or compliance roles.
