The Four Counties Nursing Team are seeking registered nurses to join their dedicated teams on a full or part time basis.
The Four Counties Community Nursing Team are based at Bourne Health Clinic and Stamford Community Hospital. They cover the towns of Market Deeping, Bourne and Stamford and surrounding villages.
As a community staff nurse you will be required to work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. Successful candidates will be responsible for assessing care needs, and the planning, delivery and evaluation of nursing care using innovative evidence-based practice.
To support you in your new role, we have developed an induction programme which allows you to learn the skills and knowledge required to be successful. The induction programme includes teaching sessions in the key areas of practice: Holistic Assessment and Care Planning, Palliative Care Awareness, Wound Care, and Medication Administration. Along with your mentor, you will work through the key areas of practice at your own pace until you feel confident and capable in undertaking visits yourself.
Main duties of the job
Community nursing is exceptionally rewarding as it allows wonderful therapeutic relationships to be developed with patients in their own homes. Working in this capacity embodies the person-centred care approach and allows patients to truly be at the centre of care delivery.
The skillset of a community nurse is vast and non-exhaustive requiring knowledge and expertise in a range of areas. Examples of interventions provided are wound management, continence care, palliative care, medication administration and long-term condition management. Close working with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patients' care is effectively coordinated demonstrates a positive impact on our patients.

About us
Lincolnshire Community Health Services NHS Trust is in the top 25 per cent of all community trusts for overall staff engagement in the 2021 NHS National Staff Survey.
At LCHS, we know diversity fosters creativity and innovation. We are committed to equality of opportunity, to being fair and inclusive, and to being a place where all belong. We therefore particularly encourage applications from candidates who are likely to be underrepresented in LCHS' workforce.
LCHS recognises the increasing demand for more flexibility in working practices in order to accommodate the personal goals and commitments that employees experience at different stages during their working lives. Flexible working opportunities such as part-time working, compressed hours, job shares, and hybrid working models can help staff to balance their personal and work commitments. Arrangements can be put in place if it can be shown that the needs of the individual can be balanced with those of the service and the impact on other team members, enabling the Trust to recruit and retain skilled staff, raise staff morale, reduce absenteeism and respond to changing service needs more effectively.
We are an ambassador of a learning culture that will support the right individual to progress in their chosen career through an 'earn while you learn' apprenticeship programme. Visit our Learning and Development page to find out more.
