Job Overview The post holder will be expected to work with the Colorectal Nursing Team within the Department of Colorectal surgery. To support the development of the nursing service, provide clinical expertise and deliver safe & effective patient centred services, which meet national, regional and local requirements for patients with a colorectal cancer and other related colorectal disease under the direction of the senior Colorectal Nurse and Lead Clinician. Main duties of the job Clinical And Professional Responsibilities T he post holder will: Deliver and promote evidence based practice and agreed care pathways To assist with and develop our Enhanced Recovery After Surgery (ERAS) pathway for Colorectal Cancer surgical patients. To facilitate effective communication with MDT to ensure care is delivered as per agreed pathways between hospital and community care providers. Triage patients daily for Straight To Test (STT) investigations. Attend Colon Capsule Endoscopy clinics. Attend and develop Patient Centred Follow Up (PCFU) clinics Identify patients, physical, social and psychological needs (Holistic assessment) in order to refer appropriately to other agencies. Ensure the provision of an integrated quality service by maintaining effective communication with the multi-disciplinary team, community services and voluntary agencies. To book new and follow up bowel cancer patients for MDT discussion as per prescribed MDT booking proforma in consultation with the clinical teams. To attend weekly colorectal MDT. When required to attend other MDT’s e.g. Lung, NET and Liver Mets in consultation with the lead nurse and lead clinician. To attend Network Cancer Group meeting (NCG) in the absence of nurse specialist.
