This will involve providingand receiving complex and highly sensitive information in relation to adults with a learning disability and working within the codes ofpractice and professional guidelines. To establish and maintain effective and safe care through the holistic and comprehensive assessment of individuals needs throughplanning, implementation, and review in accordance with policy and procedures. To complete comprehensive risk assessments and risk management plans in accordance with Health Board policy and guidance. Thepost-holder would be required to have the necessary skills to assess and interpret complex needs and the risk factors presented whenassessing those with a learning disability and the ability to recognise when positive risk management would be beneficial. To support the multidisciplinary team in developing services that are sensitive to the individual needs of adults with a learning disabilityand their families/carers, whilst taking into consideration the equality, diversity and inclusion of all involved. To provide clinically effective, therapeutically focused and evidence-based care for adults with a learning disability, both individually or ingroups, and working with families systemically where appropriate. To encourage and establish therapeutic relationships with individuals and where appropriate their relative, carers and representatives.The post holder should always ensure compliance with confidentiality and Information Governance legislation whilst requiring empathyand reassurance as appropriate. To prioritise need/demand and workload with the Team Manager, ensuring effective use of time and professional accountability. To provide health promotion on a variety of topics, in a format accessible to the individual (eg. Easy Read, Social Story). The postholderwould be required to use their knowledge and skills and work collaboratively with the multi-disciplinary team and wider colleagues withinthe health board and external agencies. To develop and maintain professional links with other agencies and to work in collaboration with individuals, their family and socialnetworks, statutory and voluntary agencies to enhance care planning and future provisions. To implement the care plan and when appropriate co-ordinate the work of other members of the Multi -disciplinary Team who areresponsible for implementing aspects of the plan. To review the effectiveness of the care plan, whilst monitoring and evaluating the quality of care provided to people with a learningdisability. Where appropriate initiate any action that may be required. To complete clear, accurate, concise and up-to-date clinical records and documentation relating to interventions undertaken This couldbe handwritten or electronically via a computerised system in line with current guidance and the professional code of conduct. To establish and maintain good links with Local Authorities, appropriate primary health care teams and other referring agencies, verballyand in writing, regarding patient care. To ensure that Safeguarding processes are adhered to in line with Social Service and Wellbeing Act. The postholder must be aware ofthe correct process to facilitate a safeguarding referral. In the absence, or at the request, of the Team Manager to chair multi-agency and multi-professional meetings with complex casemanagement. To encourage the participation of clients in the development and review of the service and any service change. Ensure close links with childrens learning disability teams and the Transition Case Manager to facilitate seamless transition for youngpeople with a learning disability into adult learning disability services, as directed by the Team Manager. To ensure the timely and accurate recording of clinical and non-clinical incidents on the Datix Risk Management system.Educational and Managerial Responsibilities: Review and request medication for therapeutic effectiveness, appropriate to patient need and in accordance with evidence-based practice,national and local protocols and guidance and within scope of practice. To manage and provide professional advice and support to junior staff with their case load aiding their development and ensuring theyare supported and a consistent approach is adopted to patient care and that policy and procedures are implemented professionally. Toalso provide day-to-day supervision and/or clinical supervision to others as appropriate. To ensure the team vision and philosophy is delivered in line with ABUHB Policies, protocols, procedures, National Guidance andResearch and is implemented by the team and all staff involved. The post-holder would be expected to consider and make commentsrelated to service improvements orientated towards improving the quality of care provided. Complete daily plans of whereabouts when away from base with all relevant contact details in line with the Health Board and local LoneWorker Policy.
