Description Suffolk County Council - West Suffolk Hospital, Bury St Edmunds, IP33 2QZ - Hybrid £33,366 per annum (pro rata if part time) 37 hours per week - Flexible working options available Permanent This advert is open to current redeployment Suffolk County Council staff only Your role and responsibilities An exciting opportunity has arisen for a Senior Independence and Wellbeing Practitioner to join the team at West Suffolk Hospital, Bury St Edmunds. The main purpose of this role is to work with customers to identify and deliver personalised good quality outcomes to the highest standards to support and maximise independence and wellbeing. Your role will include working within an acute hospital environment with the aim of supporting customers to enable their discharge from hospital. This will involve completing Care Act Assessments, assessing functional ability, carrying out Mental Capacity Assessments, promoting digital care technology and above all providing advice to customers, their families and health staff with the role of promoting and maximising the independence and wellbeing of customers. support community referrals to prevent hospital admissions – linking in with INT co-ordinators manage the referral process (including benefits tracking and recording) support communication and awareness raising manage decisions about eligibility, private pathway and charging. You will need good knowledge of Adult Social Care Services proven ability to verbally explain and describe accurately. excellent negotiating skills to liaise between customers, manager, and service providers. proven experience and understanding of a person centred, enabling approach to working with people to maximise their independence and wellbeing. The team This role offers you the opportunity to join a professional and enthusiastic team, ensuring the provision of valued quality services to vulnerable adults across the West and East of Suffolk. You will work within an acute hospital environment at West Suffolk Hospital. Whilst the majority of time is spent in the hospital environment, there may be times when home visits, care home visits and training opportunities require visits to other locations, therefore a full driving licence ability to travel is required. We offer a flexible work pattern across 7 days a week with the option of compressed hours. Some weekend working is required consisting of one weekend every four weeks and is shared amongst the team members.
