Principal Tasks
1. Engages with and follows individual residents' care plans ensuring individual, personalised support is provided in line with individuals' needs.
2. Updates residents' care plans and risk assessments with accurate relevant information in a timely manner to ensure a person-centred approach focused on the individual’s changing needs, adapting practice accordingly.
3. Uses The Maples templates and processes to capture information and maintain residents' files.
4. Offers and presents options to support and empower service user choice, reflecting the importance of appropriate risk-taking in line with care plans and goals.
5. Responsible for the capture of outcome information and data, using The Maples documentation and processes (e.g., daily records, keyworker reports) to support the review process, discussions with commissioners, and with families/carers.
6. Completes specific, delegated tasks to deadlines and quality standards in line with The Maples policies and procedures, reporting back to the senior care supervisor and/or Home Manager as agreed.
7. Supports residents within the home who need assistance with their mobility, and those with no mobility who need to be hoisted or transferred.
8. Supports residents with their daily personal care routine and continence, ensuring their dignity at all times.
9. Supports residents with their medication when appropriate training and guidance has been provided.
10. Provides Key worker support, regularly reviewing residents' information in line with The Maples policy and providing information to the individual, their significant others, and external care professionals.
11. Acts as an advocate for residents during formal meetings or situations as required.
12. Works in partnership with colleagues, appropriate family, friends, other significant individuals, and care professionals to support each individual service user.
13. Deals with issues, comments, and complaints about service delivery in line with policy and process.
14. Ensures residents are safeguarded by working within internal policies, medication management systems, staff handbook, and all relevant external regulatory requirements (CQC, etc.).
15. Responsible for your own, service user, and home visitors' health and safety (H&S) at all times.
16. Works within The Maples policies and procedures and relevant regulations at all times.
