Job Title: Social Worker – Reablement Team
Location: Brent
Rate: £32 per hour
An exciting job role has recently become available at the Brent London Borough Council. They are looking for a new member for their Reablement Team.
What will your responsibilities be?
In this role, you will conduct comprehensive assessments of individuals’ needs, strengths, and goals. You will implement interventions aimed at promoting independence and reducing the need for long-term care. Additionally, you will work closely with healthcare providers, therapists, and community services to ensure a holistic approach to reablement.

Qualifications and Experiences:
To be successful in this role, you should have Social Work England registration, be eligible to work in the UK, hold a full UK driving licence, possess post-qualified experience, and have a degree level or equivalent qualification in Social Work.
