Job summary The post holder will have continuing responsibility for the management of a defined caseload. They will be responsible for the continuity of care and be the lead person for communication to identified stakeholders. The primary function of the role is to provide proactive, responsive services that address the anticipatory and maintenance care needs of the patients/clients within the defined caseload.
