This is an exciting opportunity to join the Health Facilitation Team as a community nurse/ health facilitator - fixed term - 12 months for people with learning disabilities.
We are seeking a highly motivated enthusiastic and passionate learning disability nurse to join the Adult Learning Disability Enhancing Physical Health team, as a senior registered Nurse / Health Facilitation nurse. The Team provides specialist advice and support to both people with a learning disability and to health services to reduce the health inequalities and premature mortality that people with a learning disability face.
The team is also in the early stages of merging with the Tees Enhanced Physical Health Team.
This role is a full-time Fixed term position working hours Monday-Friday 9am-5pm.
This post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme. Please note: Newly qualified applicants or those due to qualify will be recruited via a separate process.
Within the role youare expected to participate in the Adult Learning Disability Nurse on call (Monday-Sunday 5pm-9am)
Main duties of the job
The successful candidate will work with health colleagues within the wider Learning Disability Community Teams, Local Authority colleagues and in liaison / partnership with primary and secondary care services, using a Person-Centred Approach to enable and support adults with a learning disability and their carers to promote the health needs of people with a learning disability and enable access to wider health services.
This is a vital service for people with learning disabilities who often experience difficulties in accessing appropriate and timely health care. The main responsibility of the role is to improve health outcomes and improve quality of life.
This includes:
1. Supporting with a wide range of health promotion projects (. vaccinations, cancer screening)
2. Liaison with health and social care services
3. Supporting with reasonable adjustments to access right care and support in relation to physical health.
4. Training and education for services and carers.
5. One to one support for patients who have changes in physical health and/or need support to navigate health services. Using the nursing process
6. Advice, support and signposting health and social care colleagues
7. Using contemporaneous evidence base and knowledge of health systems to navigate patient journey with regards to new or existing diagnosis.
8. Advocate and promote the needs of people with learning disabilities across health services.

10. To be recognised practice assessor / mentor or work towards in an agreed timescale
