Job summary The post holder will have responsibility for developing and supporting this speciality service to provide equitable care to patients presenting with Lymphoma Cancer and act as a resource to carers, the Multi-disciplinary colleagues and the primary health care team. This will be achieved by the post holder ensuring a smooth transition of care from initial referral through diagnosis, treatment and beyond treatment into the survivorship stage, establishing links between the acute and primary care group, supporting both patient and carers to ensure a robust stratified follow up process for cancer patients. The post holder would liaise with services within Wales and with other key partners such as Health Boards, Primary Care, Voluntary Services, Social Services and Hospices. The post holder will provide expert nursing advice and support to patients and their families at all stages of disease. The main responsibilities of the role will be related to clinical expertise with an emphasis on initiating, performing and maintaining standards of clinical excellence. Main duties of the job To deliver high quality and efficient outpatient care for lymphoma patients To assess, plan, deliver and evaluate patient care using a holistic and individualised approach, based on the principles of best practice and clinical effectiveness To provide continued support to patients throughout their journey through the outpatient setting, ensuring that their care is delivered in a timely and efficient manner To work with the Lead CNS and consultant team to develop the lymphoma service To provide patients with information, education and psychological support throughout the course of their disease and treatment The post holder will be required to work clinically The applicant would be expected to cross cover other team CNSs and take part in nurse led initiatives To maintain up to date nursing records and care plans Providing patients, their families and staff with expertise (in-depth knowledge and understanding of Lymphoma) Undertaking medicine management for lymphoma patients. Initiating and implementing standards of nursing treatment and care for Lymphoma patients Undertaking HNAs To liaise with community palliative care teams and other community based agencies to provide community support and specialist care for those that require it.
