Main duties of the job
As a member of the reablement team, you will work closely with health and social care staff, having the unique opportunity to experience both sectors. You will work under the guidance of qualified therapists, providing patient-centered interventions, and following treatment programmes to meet the goals of each service user. You will hold a small caseload of simple cases, undertaking regular reviews of their progress and altering their reablement plan accordingly to maintain a goal focus.
You will work closely with both health and social care colleagues, in a blended approach to patient care. Physiotherapists, nurses, occupational therapists, therapy assistants, and social care staff undertake collaborative working on a daily basis to ensure that all patient needs are met. Regular clinical supervision and professional support are provided.

Person Specification
Experience
* Significant experience working within a relevant clinical service as a senior HCA / HCSW
* Experience of working as an Assistant Practitioner within a relevant clinical service
Knowledge
* Working knowledge of Assistant Practitioner role within a rehabilitation setting.
* Understanding of the competency boundaries of an Assistant Practitioner role.
Skills & Abilities
* Ability to challenge poor behaviour.
* A positive attitude and able to act on feedback
* Ability to use IT confidently and competently in daily work.
Additional Criteria
* Ability to travel independently across the county
Qualifications
* Foundation Degree in Health and Social Care (or alternative equivalent award e.g. Higher National Diploma (HND) or BTEC (level 4))
* GCSE Maths and English (Grade A-C) or equivalent
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer
