Integrated Transfer Of Care Hub (iToCH) Administrator - West

We have an exciting opportunity to join our West Integrated Transfer of Care Hub (iToCH) as one of our administrators to support the hub with processing all the referrals coming into the West community service.
The Administrator will be responsible for reviewing key blockages to patient flow, highlighting key issues to Team Managers, maintaining accurate and timely feedback to referrers, and liaising with multi-agencies for the coordination of patient flow. You must possess excellent communication and people skills; have the ability to work under pressure, whilst being a committed member of our team.
This is a full-time position covering the hours of 8 am to 8 pm, 7 days/week, including bank holidays, along with the other administrator within the team.
Main Duties of the Job
The role of the Admin Support Worker is to work with prioritisers who are Nurses, Social Workers, Therapists, and other members of staff employed within the multi-professional team, processing and managing the flow of patient referrals through the Transfer of Care Hub and local point of access.
The Admin Support Worker will monitor the referrals on the electronic referral system (STRATA) on a continual basis, triaging in partnership with the multi-professional team.
The Admin Support Worker will be responsible for reviewing key blockages to patient flow, highlighting key issues to Team Managers, maintaining accurate and timely feedback, liaising with multi-agencies for the coordination of patient flow, and running reports for the system.

Person Specification
Education / Qualifications and Relevant Experience
* NVQ 3 Administration or equivalent
* Experience of working within a community setting
* Good standard of education including GCSE English & Maths or equivalent
* Previous experience in a healthcare office environment
Skills and Aptitude
* Ability to prioritise own workload and work unsupervised with accuracy and attention to detail.
* Computer literate with an excellent working knowledge of Microsoft Office, Excel, and PowerPoint.
