Job summary The remit of the service is to support the citizens of Staffordshire predominantly over the age of 18 with physical disabilities and older people over the age of 65 with physical and mental health disabilities. We need experienced and dynamic individuals who possess excellent communication and organisational skills and have a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005. The successful candidate will have a flexible approach and be a team player who is able to forge excellent working relationships with multi-disciplinary teams and partner agencies and you will be expected to be an experienced practitioner with in-depth knowledge of safeguarding and complex case management.
