DRUG AND ALCOHOL WORKER (TRILOGY OF RISK) DDAS/Choices Job Description Role Drug and Alcohol Worker (Trilogy of Risk) Service Ceredigion County Council Location: Ceredigion Barod Base Aberystwyth Hours: 21 hours per week Salary: £25,750 - £27,759 pro rata Reports to: DDAS Team leader Team Members Reporting to Job Holder: N/A Contract: Fixed term until 31 st March 2026. The Service About Dyfed Drug and alcohol service Dyfed Drug and Alcohol Service is an integrated Barod led consortium service, supported by Kaleidoscope and G4S. DDAS is the first point of contact for any adult over the age of 18 with a drug or alcohol problem in Dyfed. DDAS covers the counties of Carmarthenshire, Pembrokeshire and Ceredigion and we support service users who have drug and/or alcohol problems themselves or who are concerned about someone else’s drug and/or alcohol use. We also provide Needle Exchange Services, Dry Blood Spot Testing, Open Access assessments, Group and Coffee Mornings, SMART Recovery and much more. Porth Gofal is the integrated decision-making hub for Ceredigion that triages and determines the level of need of all cases and the best outcomes for those individuals. Porth Gofal is the heart of service provision that focuses on ensuring that Families receive the intervention that is best placed to meet their needs or where else to guide them to early help or specialist services. Role Purpose Ceredigion Trilogy of risk model is one of partnership working with DDAS and VWDAS, (specialist services) and Social Care in order to achieve the desired outcome of early help and prevention of escalation where there is concern surrounding trilogy of risk. The purpose will be to provide specialist Information, advice, and assistance to the person at the point of contact to reduce escalation of needs as support provided at the right time by the right professional. Through providing Information advice and assistance a proportionate assessment will be completed in collaboration with the family using the model of signs of safety. From this proportionate assessment the Triage will identify next steps. The aim of the role is to provide services to young people and families in relation to substance related harm by: Working in partnerships to provide comprehensive and seamless services to people using substances whatever their level of need. Work across Porth Gofal Triage and assessment service (adults/children/young people) to offer support in managing/reducing substance use. Apply the methodology of the Signs of Safety practice framework. Undertake and respond to ongoing enquiries; gathering and analysing information to inform decisions and develop professional judgement. Prescribe solutions for all identified needs. Delivering the agreed local system of triage and comprehensive assessment appropriate to the presenting need. Training and supporting Tier 1 & 2 services to identify at risk children and young people with regards to Hidden Harm. Targeting high risk and local priority groups. Engaging people with issues relating to their own substance use. Reducing substance related harms to families. Improving and supporting family’s motivation for change. Appropriately assessing and allowing individualized care pathways to be developed with the whole family. Providing a range of evidence-based interventions. Supporting people to maintain or build positive social networks. Ensuring all people are contacted if they disengage with the service. Signposting identified populations affected by drug and alcohol use to appropriate services where they fall outside of the remit of this specification. Involving families and carers in the treatment of individuals where appropriate. Maintaining good working relationships with key partners across all Tiers in particular DDAS, Choices, CDAT, Social Services, and West Wales Domestic Abuse services. Deliver person-centered, outcome focused work with individuals, families, careers, groups and communities that promotes independence, wellbeing and social inclusion. Assist service users, families, and carers to participate in any assessment processes and provide them with appropriate information to enable them to make informed choices about those services available. Main Duties and Responsibilities In this role you will be responsible for the following: To complete assessments ( IA )including risk assessment and risk management planning. To be able to identify other areas of concern surrounding Tribology of risk and share appropriately to other work colleagues. To provide crisis interventions and advise staff on appropriate information and advice. To sign post to mutual aid and peer support groups for young people and their family affected by someone else's substance use. To provide brief interventions that consist of short-term focused work to address immediate problems. To provide concerned others with a forum to seek support, encouragement and learning opportunities from each other. Promote healthier lives, wellbeing, and active citizenship. Promote and encourage service user participation in local, regional, and national family and carer events. Advocate on a young person’s behalf. Provide training or talks to internal staff or external organisations. Promote self-help guidance and digital inclusion provision. Ensure that all safeguarding requirements are understood and adhered to. Support the service to provide and manage the single point of contact. To work within agreed policies, procedures, and protocols to safeguard children and vulnerable adults. To attend and report to safeguarding reviews, meetings, and conferences e.g., Multiagency Risk Assessment Conferences (MARACs) Work in a flexible and responsive manner. Prepare individuals to support clinical treatment. Providing advice and information and assist service users into structured services. To provide accurate advice and information regarding substance use and associated lifestyle behaviours that reduces the risk of overdose, reduces the spread of blood borne viruses, promotes healthy living, and minimises the potential risks and impact of drug related crime and disorder on the wider community. To work in partnership with other agencies to provide a package of support around holistic needs, by attending joint allocation meetings and case review meetings, sharing appropriate information, and ensuring that action plans are carried through. Provide ad-hoc support for complex cases for those cases who are being care coordinated by a Social Worker. Liaise and signpost with external agencies including, but not limited to, specialist substance use services, mental health, sexual health, housing, debt, and finance. Inspire and enthuse individuals to achieve and maintain positive lifestyles changes. Any other duty or task deemed appropriate by line manager Outreach and Harm Reduction Provision Deliver harm reduction services and advice, including provision of Naloxone Training, Needle Syringe Programme (NSP) and Dry Blood Spot Testing (DBST). To organise displays, exhibitions and participate in community and agency led events where appropriate. Compliance and Information Management Ensure appropriate service user records and case notes are completed and maintained. To ensure the undertaking of appropriate monitoring, review, and evaluation procedures. Adhere to safeguarding and data protection principles in working practice. Adhere to host organisation and Ceredigion County Council policies and procedures. Maintain appropriate boundaries. Engage with Learning and Development Plans to ensure training remains up to date. To keep up to date with strategic and policy developments and emerging trends in respect of substance use and related matters. Undertake appropriate administrative tasks. Service Development Support the continued delivery of a high-quality service in line with the service specification and regional delivery plans. Essential Desirable Qualifications and training Full, current driving licence and use of a vehicle which is insured for business use. Previous experience within the field of youth work, education, training or family support. An understanding of current legislation and policy in relation to substance use and mental health for children and young people and their concerned others. GCSE English Literature or Language, grade C or above (or equivalent). Health and Social Care Diploma Level 3 (or equivalent). A suitable qualification in training or education. A suitable qualification or training in IT proficiency. Role-related knowledge, experience and skills A successful track record of providing support to young people and their concerned others, from vulnerable or marginalised groups / extensive experience of delivering substance use advice and support. Experience of assessing, planning, and delivering a range of evidence based, therapeutic family and carer interventions which are part of a planned, structured programme. Experience of providing skills-based structured interventions, triage, and crisis management to concerned others. The ability to promote and support the attainment of recovery capital for those children, young people and concerned others, affected by mental health and/or substance use. Understanding the barriers, pathways, and opportunities available for YP including training, skills, and volunteering. Practical and up to date knowledge on the principles of data-protection and confidentiality, in relation to working with young people and data. The ability to compile, maintain and complete accurate written records, including data capture and the use of case management systems. Experience of delivering a range of activities to children, young people and concerned others. Ability to manage and lead group work sessions.
