Job Purpose:
We are seeking a skilled and experienced Agency Social Worker to join our Hospital Discharge Team in Powys. The role involves working closely with health professionals and service users to ensure the safe and timely discharge of adults from hospital settings. You will assess needs, coordinate care, and ensure that service users have the appropriate support in place to enable a smooth transition from hospital to home or other care settings. This is a temporary position with competitive agency rates, offering flexibility for social workers seeking contract work.
Key Responsibilities:
Assessment and Discharge Planning: Undertake holistic assessments of service users' needs to ensure a safe and well-coordinated discharge from hospital, considering both their immediate and long-term care needs.
Care and Support Planning: Develop tailored care plans in collaboration with service users, their families, and other health and social care professionals to support the transition from hospital care to home or community-based services.
Collaboration with Health Partners: Work closely with hospital staff, including doctors, nurses, occupational therapists, and physiotherapists, to ensure a coordinated and person-centered approach to discharge planning.
Safeguarding and Risk Management: Identify and respond to safeguarding concerns, implementing protective measures in line with local authority safeguarding procedures.
Mental Capacity Assessments: Conduct mental capacity assessments as required, ensuring that decisions about care are made in the best interests of service users in line with the Mental Capacity Act.
Crisis Intervention: Provide immediate support in urgent situations, ensuring that service users in crisis receive timely interventions and support post-discharge.
Post-Discharge Support: Arrange for ongoing care services, including domiciliary care, reablement services, equipment provision, and residential placements where needed.
Multi-Agency Working: Liaise with community health teams, housing services, and third-sector organizations to ensure that service users receive a seamless and integrated service following discharge.
Record Keeping and Compliance: Maintain accurate and timely records of assessments, care plans, and actions taken in accordance with local authority policies and statutory requirements.

A degree in Social Work (or equivalent).
