Job summary
Bramhall Health Centre are looking for a practice nurse toassist medical personnel in the care of patients to include treatment,preventative care, screening and patient education. The position plays a vital role in building strong relationships with patients with long term conditions and effectively engaging patients with their treatment and long term management plans.
Main duties of the job
Assistin and perform routine tasks related to patient care as directed by GPs
Cytology
NewPatient Medicals/Urinalysis
TravelClinic including travel advice and vaccinations
ECGs
Spirometry
Venepuncture
HypertensionManagement
Routineimmunisations/Childhood immunisations
Chaperoningand assisting patients where appropriate who are being examined by anotherclinician
Requestingpathology tests, for example urine culture, swabs
Chronicdisease management including diabetes, asthma and COPD
Followingagreed clinical protocols with referral to GPs as appropriate
Home visits to carry out vaccinations and chronic disease management
Assist with chronic disease management within allocated care home
Othertasks will include:
Maintainingand cleaning equipment used by the nurses and GPs
MaintainingGP and Nurses rooms, stocking and rotating items as required
Chaperoningand assisting patients who are being examined by another clinician.
Maintaingeneral tidiness and cleanliness of nurses and treatment rooms
Orderingof Health Promotion Literature
Orderingof vaccinations to maintain stock levels
Participationin administrative systems in the Practice
Atall times there will be a need to maintain accurate records
Attendand participate in any Practice meetings when required
Anyother delegated duties appropriate to the post

Lead by 4 partners and 2 managers, the wider team consists of 4 salaried doctors, 1 Advanced Nurse Practitioner, 1 Practice Nurse, 1 Nurse Associate, 2 Healthcare Assistants and 20 admin staff across reception, admin, secretarial and prescription departments. We also love our local Primary Care Network consisting of 5 practices which delivers enhanced services giving us the opportunity to work with a physio, a team of pharmacists, a mental health practitioner and a variety of care coordinators which work with our patients for long term conditions.

Job responsibilities
Providingassessment, screening and treatment services and health education advice
Organise and co-ordinate the provision of nursing services forthe Practice to include the delegation of duties to other Practice nursingstaff as appropriate.
Provide nursing treatments to patients in participation withgeneral practitioners or independently to agreed protocols.
Provide general and specific health screenings to the Practicepatients (within agreed protocols) with referral to general practitioners asnecessary.
Advise patients on general health care and minor ailments withreferral to GPs as necessary.
Supplies and equipment Treatment room and other areas
In participation with the Practice Manager, ensure themaintenance of equipment and stock levels relating to patient care.
Pathologicalspecimens and investigatory procedures
Following appropriate training, undertake the collection,storage and despatch of pathological specimens including intravenous bloodsamples, swabs, smears etc and ensure the cleanliness of any equipment soused. Perform any investigatoryprocedures requested by the GPs.
Administrativeand professional responsibilities
Participate in the administrative and professionalresponsibilities of the Practice team.
Ensure accurate notes of all consultations and treatments arerecorded in the patients notes on the clinical computer system as appropriate.
Ensure accurate completion of all necessary documentationassociated with patient health care and registration with the Practice.
Ensure collection and maintenance of statistical informationrequired for regular and ad hoc reports.
Attend and participate in Practice meetings as required.
Assist in the formulation of Practice philosophy, strategy andpolicy and develop appropriate protocols.
Maintain a notice board in the waiting area designated forpatient health care education.

Qualifications
Essential
1. Nursing degree
2. Registered Nurse with accredited nursing body
3. At least 12 months relevant experience in general practice environment
4. Knowledge of Emis and Docman systems
5. Certificate of competencies in cytology, immunisations and phlebotomy
6. Competent in management of Diabetic, Asthmatic and COPD and management
