Detailed job description and main responsibilities
The "must haves" for you to be considered for this role:
1. Health related qualification E.g. NVQ /QCF Level 3-5 in Health & Social Care or higher OR Significant equivalent relevant experience in relevant settings together with relevant short courses.
2. A passion for physical health care that matches our passion to provide excellent healthcare for people with serious mental Illness who may also have physical comorbidities.
3. A Registered Nursing Associate or Assistant Practitioner with experience of working with people with SMI and / or long-term conditions and the ability to undertake phlebotomy, ECG and vital signs recording & monitoring.
4. You will be comfortable working in a busy environment and proficient at engaging with people who experience health inequalities or difficulty accessing services.
5. You will be required to have access to your own vehicle and a valid driving licence enabling you to travel to clinics and patients' homes.

Education/Qualifications
Essential criteria
1. Health related qualification E.g. NVQ /QCF Level 3-5 in Health & Social Care or higher.
2. OR Significant equivalent relevant experience in relevant settings together with relevant short courses.
3. Commitment to undertake further required training, professional development and self-improvement to support patient care and experience.
4. Trained in phlebotomy (or willingness to undertake training).
5. Trained in undertaking ECG's (or willingness to undertake training).
Desirable criteria
1. Nursing Associate qualification.
Previous Experience
Essential criteria
1. Robust and relevant experience within healthcare settings.
Desirable criteria
1. Experience working within community outpatient mental health settings and/or long-term conditions.
Knowledge, Skills & Abilities
Essential criteria
1. Ability to communicate effectively orally and in writing at all levels including with patients, carers and MDT members/Trust employees.
2. Demonstrable understanding of a Mental Health Act and Mental Capacity Act.
3. Able to work effectively with colleagues, patients and their families.
4. Able to work under pressure.
5. Ability to understand and evaluate risk to maintain patient safety.
6. Honest, trustworthy, reliable.
7. Understand the limits of your abilities.
