Main Duties of the Job
As a GP in Stalybridge, you will provide patient consultations for both acute and routine cases, managing a varied caseload. You will care for patients with minor ailments, chronic diseases, and undifferentiated medical conditions while ensuring high-quality, protocol-driven care.
You'll handle referrals by assessing patients and making onward referrals to specialists or secondary care, working collaboratively with the referral management team.
For prescription management, you will review, sign off, and process prescriptions, supporting other GPs with medication reviews, repeat requests, and resolving prescription-related queries.
Collaboration is key, and you will work as part of a multi-disciplinary team including Nurses, Pharmacists, HCAs and Patient Advisors. You'll also contribute GP input and leadership to support patient care.
Job Responsibilities
Key Responsibilities:
* Patient Consultations:
o Conduct consultations with both acute and routine patients.
o Manage a varied caseload, including minor ailments, chronic diseases, and undifferentiated medical conditions.
o Ensure patient care adheres to the clinical guidelines and practice protocols.
* Referrals:
o Assess and triage patients, making appropriate onward referrals to specialist services or secondary care when necessary.
o Collaborate with referral management teams and other healthcare professionals within the PCN to ensure seamless patient care.
* Prescription Management:
o Review and process prescriptions efficiently, ensuring compliance with best practice and clinical guidelines.
o Support other healthcare professionals in the PCN with medication reviews, repeat prescriptions, and resolving medication-related queries.
* Collaboration with the Practice Team:
o Work as part of a multi-disciplinary team that includes nurses, pharmacists, healthcare assistants, nurse practitioners, administrative staff, and GPs.
o Participate in team meetings, case discussions, and quality improvement initiatives.
Person Specification
Essential Skills and Abilities
* Clinical Competence: Strong clinical skills, including the ability to assess, diagnose, and manage undifferentiated medical conditions, as well as routine and acute patient care.
* Referral Management: Ability to make appropriate onward referrals to specialist services or secondary care.
* Prescription Management: Competent in reviewing and processing prescriptions, adhering to clinical guidelines.
* Communication Skills: Excellent written and verbal communication skills for patient consultations and teamwork.
* IT Proficiency: Competent in using electronic health records system (EMIS Web) and other digital tools for patient care and prescriptions.
* Commitment to continuous improvement and learning through reflective practice, mentorship, and attending relevant courses and seminars.
Essential Experience
* Newly Qualified GP: Less than 2 years of post-qualification experience working as a GP.
* Primary Care Experience: Experience in primary care settings during GP training or post-qualification.
* Patient Management: Proven ability to manage a variety of patient cases, including acute, routine, and chronic conditions.
Essential Qualifications
* GMC Registration: Full registration with the General Medical Council (GMC) and a license to practice in the UK.
* MRCGP Qualification: Membership of the Royal College of General Practitioners (MRCGP), or equivalent qualification.
* Right to Work in the UK: Eligibility to work in the UK without sponsorship.
