Job summary
Colne Valley Primary Care Network has an exciting opportunity for a hardworking and enthusiastic INT enhanced practice nurse to join our team. You will be required to travel between GP practices within our PCN and this role is offered on a part-time basis, for 30 hours per week.
The post holder will have critical thinking and be a caring, compassionate, committed, experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
We are developing a 'Neighbourhood' model as per the vision in the Fuller Stocktake report, with a focus on preventative and proactive care and helping patients see the right clinician at the right time.
Main duties of the job
1. Demonstrate critical thinking in the safe clinical decision-making process.
2. Spearhead the Integrated Neighbourhood Team across the PCN.
3. Communicate and work collaboratively with the general practice teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required.
4. Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
5. Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.
6. Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
7. Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
8. Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
