Job summary An exciting opportunity has arisen in the Older Persons Community Treatment Team Newcastle/North Tyneside, for a Band 6 Community Practitioner (Nurse, Social Worker or Allied Health Professional) based in the Centre For Aging and Vitality (CAV) on the old Newcastle General Hospital Site. We are seeking a highly motivated clinician to undertake assessment and offer treatments. Experience in working with older adults is required. The post holder will provide assessment and treatment function, this would be across a variety of environments e.g. community care home or patient's own accommodation. The post holder will be a Care Coordinator working not only with the Mutli Disciplinary Team but also working with other agencies and voluntary sector You will possess excellent interpersonal and communication skills. You will have the ability to work both independently and within a team. You will demonstrate the ability to liaise with different agencies. You will need to be skilled and comfortable in dealing with clients who have multiple and complex needs, including issues around risk management.
