Care Station NW is seeking a Care Coordinator to join our established office in central Blackburn in a full-time permanent position. The successful candidate must have:
* Experience of working as a Care Co-Ordinator/Scheduler (Essential) or other senior care role such as deputy manager.
* 2 years’ experience in a care setting.
* Hold or be working towards level 4/5.
* Exceptional organisational skills are essential for this role.
* An analytical/data-driven way of working.
* Full Driving License and own car, as this role will require travel.
Key Responsibilities for the Care Coordinator role:
1. Creating and managing schedules for our team of staff to ensure clients receive timely and consistent care.
2. Communicating with clients and their families to establish care requirements, preferences, and any changes in scheduling.
3. Assigning and coordinating staff to clients based on their needs and preferences.
4. Maintaining accurate and up-to-date records of schedules, client information, and staff assignments.
5. Addressing scheduling conflicts and adjustments promptly and effectively.
6. Collaborating with staff and healthcare professionals to ensure seamless care delivery.
7. Developing and managing client relationships within the home care team.
8. Managing a team of homecare staff.
9. Completing monthly reports and care package schedules within deadlines.
10. Managing quality monitoring of staff, supervisions, and appraisals.
11. Conducting quality assessments, risk assessments, and monitoring of service users.
12. Participating in the on-call rota (1 in 3 weeks).
13. Adhering to the policies and procedures of Care Station NW.
You must be approachable with excellent customer service skills and experience. You will be a clear communicator with excellent telephone manners and highly organised and self-motivated.

Salary: starts at £12.50 moving up to £13.17 on completion of 6 month probation.
