We are looking for a Care Coordinator to join our growing team in Sefton and play a vital role in delivering exceptional care to our clients. Role Overview: As a Care Coordinator, you will be responsible for coordinating and managing care services for our clients. This key role ensures that our clients receive the highest standard of care tailored to their specific needs. You will oversee scheduling, support the team of caregivers, and act as the first point of contact for both clients and staff. The role will involve providing on-call support outside of regular working hours. Key Responsibilities: Care Planning & Coordination : Develop and maintain personalized care plans for clients, ensuring they meet individual needs, preferences, and goals. Scheduling & Staff Management : Schedule caregivers to ensure timely and appropriate client care. Manage and maintain accurate records for all shifts and staff members. Client & Family Communication : Maintain regular communication with clients and families to ensure satisfaction with care. Act as a contact point for concerns, queries, or feedback. On-Call Support : Provide on-call support during evenings, weekends, or outside normal working hours on a rotating basis. Caregiver Support & Supervision : Offer guidance and support to caregivers, ensuring adherence to care plans and best practices. Conduct regular performance reviews and provide feedback. Compliance & Quality Assurance : Ensure care delivery complies with regulatory standards and company policies. Monitor and report on care quality. Documentation : Maintain accurate records of care plans, schedules, incidents, and client needs. Ensure timely updates to client records. Skills & Qualifications: Previous experience in care coordination or a similar role within the care industry is preferred. Strong communication skills, with the ability to liaise effectively with clients, families, and staff. Excellent organizational and time management skills. Compassionate and person-centred approach to care. Ability to manage multiple priorities and problem-solve under pressure. Good understanding of regulatory requirements and care standards. Full UK driving license and access to a vehicle is essential. Willingness to be flexible and provide on-call support as part of a team rotation. Why Work With Us? Competitive Salary : £21,950 per annum. Working Hours : Full-time - 32.5 hours per week, 9:00 AM – 4:00 PM, Monday to Friday (on-call shifts will be additional as part of the role).
