Job summary
As a member of the GP Practice Pharmacy Services Team the Pharmacy Technician supports the development and delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.
Working alongside the Senior Pharmacy Technician and with Clinical Pharmacists, to contribute to high-quality, cost-effective and safe medicines management. This will include medicines reconciliation post discharge, transcribing from clinical letters as well as monitoring near patient testing for patients taking high risk medications.
Main duties of the job
You will berequired to undertake the CPPE pathway which will equip you with thenecessary knowledge, skills and experience to work in a patient-facing role inprimary care networks, as part of a multidisciplinary team. The Pathway willtake 15 months to complete.
As acandidate you will need to have a minimum of one year experience as a qualifiedand registered Pharmacy Technician. Experience in General Practice and workingwith EMIS and Docman would be desirable but is not essential.

Job responsibilities
1. KeyResponsibilities:
1. Accurate transcription of medications fromhospital discharge summaries and clinical letters
2. Conducts medication use reviews independentlywithin own competence including reauthorizing eRD
3. To undertake medicines reconciliation in accordancewith practice protocols and to update patients medical records accordingly. Indoing so ensures maintenance of accurate patient records and that all relevantinformation is documented in the patients medical notes in a timely manner
4. Supports the Practice Based Clinical Pharmacistto provide medication review services to patients via clinics in the practiceand telephone consultations.
5. To respond to medication queries from patientsand staff in a professional manner within defined competencies only referringto the appropriate Clinical Pharmacist or GP in accordance with practiceprotocols. This involves using resources such as the BNF, SPCs, CCG MedicineManagement Guidelines and NICE guidelines.
6. Assists with the training and development ofpractice based receptionist / prescribing clerks and administrators to supportimproved co-ordination and effective pharmacy administration within thepractice, especially in relation to repeat prescribing systems and processes.
7. Liaising with local pharmacies regardingprescription queries to help ensure optimum therapy for patients as a result.
8. To provide advice to GPs, staff and patients inchanges to prescribing to support the improvement of prescribing safety,quality and cost effectiveness.
9. Conducts medication use reviews independentlywithin own competence
10. Promote cost-effective, safe, evidence basedprescribing in accordance with local and national Medicines OptimisationStrategies
11. To participate in medication audits.
12. To support the achievement of the practicesprescribing Quality and Outcomes Framework targets and any other medicinerelated enquiry or audit.
13. To advise on the sourcing and safe management ofmedicines as appropriate.
14. Participate in practice multidisciplinarymeetings, patient participation groups, and other meetings to improveengagement of the role of the pharmacy technician within the practice and topromote issues relevant to prescribing and medicines optimisation
15. To keep professionally up-to-date at all timesand to meet the General Pharmaceutical Council standards for continuingprofessional development so as to maintain professional registration.
16. Be capable of prioritising work whilstmaintaining accurate and timely records

18. BTEC /NVQ level 3 or equivalent in pharmaceutical sciences
19. Current registration as a pharmacy technician with the General Pharmaceutical Council (GPhC)
20. Evidence of continuing professional development

21. Postgraduate qualifications of a relevant nature. ACT / Medicines Management
22. Intermediate level Microsoft Office
