Main duties of the job
To work within the framework of a multi-disciplinary team to provide support and guidance to adults with learning disabilities and their carers. To develop skills and knowledge within the framework of level one and two health facilitation.

To manage an active caseload, maintaining regular visits to all allocated patients with appropriate supervision.
To focus on individual health outcomes for people with learning disabilities and offer support to access health services.
