Waythrough launched in October 2024 following the merger of Humankind and Richmond Fellowship. Our vision is to break down the barriers that stop people getting the support they need to live a life they value. We tackle poverty and disadvantage in communities, through mental health, drug and alcohol, housing and related support.
To work as part of a team to provide specialist support for LGBT+ young people and their families, in particular around their gender identity or sexual orientation.
Key accountabilities:
1. Manage a caseload of service users as allocated by the Lead Practitioner or Team Manager and complete high quality and comprehensive assessments and risk assessments, including liaising with family members, significant others and professionals/agencies as appropriate.
2. Develop comprehensive support plans to identify how individual service user needs will be met in order to help each service user live a fulfilled and healthy life free from stigma and achieve sustained outcomes.
3. Review individual support plans within appropriate timescales and no later than once every three months.
4. Deliver support sessions to small groups of Parents/caregivers to increase knowledge of specific LGBT+ issues relating to their child/young person.
5. Deliver sessions to professionals to raise awareness of LGBT+ issues and the service, to encourage referrals, increase understanding and enable LGBT+ young people to be supported.
6. Work as part of a team providing cross provision/service support where reasonable and attending regular team meetings.
