About The Role

We have an exciting new opportunity available for a Band 6 – 7 nurse with a minimum of 2 years of clinical experience in adult nursing.
The role is for a full time nurse, with flexibility of working from home. You will cover our Southern region which currently consists of two registered CQC locations: London and Essex.
Responsibilities:
1. To conduct clinical assessments and produce detailed care needs assessments for our service users to ensure they can remain safely in their own homes and meet their personal outcomes.
2. To work in partnership with the care team to provide clinical support and information to families and professionals.
3. Care planning including individualised risk assessments where required.
4. Medication management.
5. Taking clinical responsibility for a caseload of complex clients, which will include new packages, reviewing current care packages and future care packages.
6. Have a clear understanding of personal accountability, undertake clinical procedures and training with individuals in accordance with care plans and sign off required competencies in a timely and safe manner.
7. To provide training, competency sign off for staff clinical training, clinical competencies and its documentation.
8. Providing feedback to the Registered Manager.
9. Providing theory and practical training as well as competency sign off for various clinical interventions to all care staff.
10. Recognise and respond appropriately to urgent and emergency situations.
11. Record, audit and complete any appropriate documentation and records on systems in order to ensure that individuals’ records are accurate and informative.
12. Apply professional skills and manage own professional competence and accountability, in accordance with the NMC Code of Conduct.
13. Keeping up to date with best practices in line with NMC and supporting healthcare staff with revalidation.
14. To support in emergency situations on-call.
15. Oversight of all clinical practices.
16. Be able to hold clinical review meetings with CCGs.
17. Support RM with any clinical safeguarding, complaints, issues, etc.
18. Able to hold clinical review meetings with RM to discuss best practice and training needs.
19. 6-8 weekly patient safety review meetings where we can discuss all packages; clinical practices, any safeguarding, medication issues and share good news.
20. Keep up to date with changes in legislation.
21. Ensure all company clinical policies and procedures are up to date and reflect the current guidelines.
22. Conduct internal audits of MARS records.
23. Have a presence at CQC inspections and Council/CCG audits if required.
About You
Experience Required:
1. Registered Nurse Band 6 – 7 with a minimum of 2 years post clinical experience in adult nursing.
2. Strong communication skills.
3. Proven experience working with complex clients.
4. Ability to prioritise case by case.
5. Full, clean driving license.
6. Ability to work well within a team and on your own.
7. IT literate with MS Office and willingness to learn our electronic call monitoring systems.
8. Full understanding of the NMC Code of Practice.
9. Experience in delivering clinical training.
10. Good understanding of CQC domiciliary care standards.
11. ITU experience.
