We are seeking a dynamic and proactive Site Capacity & Flow Lead to join our team based at St Richards Hospital in Chichester. In this vital role, you will provide effective line management and/or leadership of designated staff at St Richards Hospital operating as part of the TOCHs (Transfer of Care Hubs).
The Site Capacity & Flow Lead will provide clinical leadership and expert advice to staff pertaining to all matters of discharge/transfer to ensure high quality and safe patient flow with specific responsibility at St Richards Hospital for identifying and facilitating the "pull" of patients in pathways 1 and 2 directly into SCFT community services.
The Site Capacity and Flow lead is a key member of the TOCHs and will work closely with the Capacity and Flow Head of Service and the TOCH leads to develop a strategy to improve patient experience and outcomes of discharge/transfer by focusing on system wide data and patient and staff feedback.
The role involves managing and coordinating a wide range of complex and competing priorities regarding discharge/transfer and necessitates working closely with a range of health and social care system partners.
To deputise for the Capacity and Flow Head of Service when required.
Main duties of the job
1. Communication and working relationships
a) To develop excellent relationships with all key internal and external stakeholders including community and acute bed managers and system level partners in acute provider services and social care system wide.
b) Develop good working relationships with all partner organisations, to gain support and contribute to the development of system wide improvements in patient discharge/transfer and flow.
c) Provide regular training, guidance and leadership to all staff, including ensuring that any changes to systems and policies are implemented in a sustainable way. Working closely with partner organisations to support the delivery of national and local operational performance standards pertaining to patient discharge/transfers from the acute Trust.
d) Ensure that key risks to delivery are identified and actions are taken to mitigate these risks. If beyond the post holder's scope of authority to manage then know and follow the Trust escalation policies.

Job responsibilities
1. a) To lead, oversee and drive forward effective and efficient patient flow, ensuring continuous improvement across pathways 1-3 at St Richards Hospital.
2. b) Provide a senior operational interface with other senior staff within the health and social care system.
3. c) To lead a site-based Transfer of Care Hub with the authority and capability to make decisions with multiple organisations to maximise the quality, efficiency and effectiveness of all hospital discharges/transfers.
4. d) To produce TOCH updates for the named individual site, being the first line of escalation to system challenges and pressures including producing appropriate performance reports for the system.
5. e) Lead on the named site operational and performance meetings including daily calls/huddles and being the identified SCFT lead for site MADE events (for all pathways).
6. f) To work closely with matrons and managers of in-patient Intermediate Care Units, Urgent Community Response teams and Virtual Ward teams to ensure optimal use of capacity ensuring patients are transferred to the most appropriate SCFT community service. This includes having responsibility for mapping patients into available SCFT beds and (where appropriate) Brighton & Hove contracted beds.
7. g) To contribute to the development of the Trust's operational policies relating to patient flow and discharge.
8. h) To interpret and translate into operational practice national and local best practice guidance with respect to maximising patient flow.
9. i) To work collaboratively with external stakeholders and partner organisations to address urgent care local pressure, system surges and escalation ensuring capacity is fully utilised.
10. j) To support ward staff and patients and their families to agree discharge/transfer plans providing practical solutions to prevent unnecessary delays.
11. k) Provide key leadership and effective line management of designated staff within the Capacity and Flow team.
12. l) To be responsible for the financial management of an identified Capacity and Flow team budget.
Person Specification
Qualifications and/or Professional registration
* Current NMC registration, or other Health Care Professional qualification
* Evidence of CPD management qualification
* Evidence of continuing professional development
* Up to date NHS policy initiatives, changes and developments
* Post graduate diploma or masters equivalent knowledge gained through experience
Experience
* Experience of operational management
* Knowledge and experience of Bed Management and Discharge Planning
* Experience of working with the wider health and social care system, including acute providers, to develop systems of care that respond to requirements
* Experience of delivering challenging performance agendas and ability to demonstrate significant performance improvements
* Experience of working with a defined performance framework set by commissioners and regulators and delivering required performance standards
* Experience of managing and leading change in complex operational environments
