The post is a senior clinical post with accountability for clinical effectiveness and service development/coordination in response to the health needs of those with complex learning disabilities. The role includes working with individuals to understand their health needs and to plan appropriately for these. The role will cover holistic care, which includes both physical and mental health aspects. Case management and coordination of support, assessing and reviewing needs in a person-centred approach. Liaising with other agencies and providers to ensure reasonable adjustments are in place.

Main duties of the job
As part of the multidisciplinary team, you will be responsible for assessing, planning, and managing the risk of individuals' care needs, including physical, mental health, and behaviours that challenge services.
Case management and care coordination for those that are fully health funded. Liaising with commissioners in conference regarding the identified health needs and how they are being addressed. Speaking up for individuals with a learning disability and ensuring reasonable adjustments are made by generic services to allow the service user to have equitable health care by the most appropriate service. Liaising with other agencies, providers, and families in partnership with the individual and keeping in mind the safeguarding process. Ensuring consideration of capacity and restrictive practice in line with best interest and court of protection processes. To be an autonomous practitioner and be able to prioritise workloads to meet the demands of the service and the wider Livewell SW objectives.

Clinical Responsibilities
1. Participating in the undertaking of health needs reviews and health assessments to identify needs and plan care accordingly.
2. Identifying, planning, and managing risk of individuals' care needs, relating to physical health, behavioural, and/or mental health.
3. Responsible for assessing, planning, and making recommendations for future care in partnership with the patient, carer, and multi-professional agencies including case conferences for reviewing the packages of care and care coordinating as appropriate.
4. Liaising with Primary and Secondary care agencies to ensure better health outcomes for individuals.
5. Working within the Mental Capacity Act and evidence best interest decision making.
6. Maintaining effective and appropriate communication networks with members of the multidisciplinary team, the independent sector homes, and all stakeholder agencies and organisations to ensure that individual care needs can be met.
7. Contributing to multidisciplinary team specialist assessments.
8. Ability to work as an autonomous practitioner, planning, prioritising, and organising workload according to the needs of the service.
9. Providing relevant information and explanation to clients and carers at a level commensurate with their ability to understand using total communication.
10. Providing evidence-based advice and guidance to care staff on the management of health needs to patients and signposting to other services.
11. Acting as a supervisor and/or assessor for students demonstrating high standards of practice and professional conduct.

5. Communications and relationships
The post holder will be required to work within the MDT function of the CLDT and may need to be part of wider MDT groups relating to client needs.
The post holder will liaise between other health, social, and educational services in assessing, planning, and reviewing client care needs/service provision.
The post holder will also work with other voluntary and secondary care providers.
