We have an opening for you to join us as a Support Worker within the Care and Support Team. The purpose of the role is to provide autonomous assistance to the people we support to fulfil their potential for independence, choice, inclusion, and control.
You will be responsible for ensuring that care and support is of a high standard, focused on person-centred outcomes and delivered within budget.
You will ensure that all needs assessments, support plans, and risk assessments are kept up to date. There will also be direct engagement and liaison with social workers, family members, and key workers as required.

Croft Community is a vibrant community located on the edge of the North Yorkshire market town of Malton. We combine individualised and shared support, enabling people to access the community’s broadest possible range of opportunities. We have a variety of places for people to live, including large, medium, and small shared houses or individual self-contained flats.
We offer a programme of activities and supported work opportunities, including Kingfisher Café, a social enterprise supporting people with learning disabilities to gain experience in a busy café environment.
At Croft, we also have a textiles workshop, which offers a range of traditional and modern craft activities, including woodcrafts, weaving, felting, knitting, and sewing. The people we support rediscover old skills and learn new ones.
At our market garden, we grow fruit, vegetables, and herbs, providing Croft Community members and Kingfisher Café with delicious homegrown fresh produce.
Duties & Responsibilities
Main Responsibilities:
* Working under the direction of the Team Leader, you will plan and provide support and care to foster the wellbeing and development of the people we support in accordance with their person-centred support and care plan, recording and reviewing progress as directed so that the people we support can fulfil their potential for independence, choice, inclusion, and control, supporting them to:
o Encourage the people we support to maintain a clean, tidy, and welcoming living environment.
o Support them to use appropriate laundry equipment.
o Plan, shop, and prepare healthy meals alongside the people we support.
o Assist them in arranging and attending medical appointments.
o Support them in managing their finances.
o Encourage tenants to maintain an active social life.
* Provide personal care where required and appropriate as part of their support and care plan.
o Administer and record medication.
o Plan meals based on individual preferences.
o Maintain related paperwork including support plans and risk assessments.
* Ensure tenants receive their housing-related support.
* Work within the community’s policies and procedures, promoting equality and diversity.
* Undertake training and continuous professional development.
* Work in close partnership with family members and other professionals.
* Undertake personal development to understand our philosophy and values.
Person Specification
Knowledge & Experience
* Experience of supporting adults with learning disabilities, autism, and distressed behaviours/emotions is preferred, but training will be provided.
* Driving License is desirable but not essential.
* Sleep-ins are required for this role.
