As part of this Multi-disciplinary team you will support patients in their own home under our 'Hospital at Home' philosophy. This will be a combination of both nursing and therapy tasks under the supervision of registered professionals, to enable patients to receive the right care at the right time in the right place. Hours per Week: 37.5 (Part time hours will be considered) Main duties of the job To contribute and support the delivery of high standard of effective care, as part of a multidisciplinary team, in support of and under the direction of a registered professional To carry out assigned tasks, including direct nursing care and administrative duties as delegated by the nurse in charge You will ensure that patients/clients receive safe, individualised and effective care which supports health & wellbeing and promotes self- care where appropriate and possible. To work with an individual patient, guided by the client's individual needs and directed by the relevant member of the multidisciplinary team. To provide care to people in their own homes following a period of acute illness or injury, who require a period of rehabilitation to enable them to return to their previous level of function. To provide support for patients who are in the palliative stage of their illness and their family, allowing for the patients preferred place of care or death at end of life.
