Occupational Therapist, Ledbury Intermediate Care Unit - 12 month Fixed term - Secondment cover. 18.75 hours per week.
We have an opportunity for an Occupational Therapist to join and work alongside other members of the multi-disciplinary team based at Ledbury Intermediate Care Centre. This role is a mixture of working in the intermediate care unit and in people's own homes as part of the discharge to assess pathway.
The team's aim is to develop optimum levels of function and independence in the physical, emotional, and social aspects of life and to facilitate discharge, prevent further admission to hospital or long-term care, and to reduce any inpatient length of stay, supporting the principles of care closer to home.
To work autonomously on the unit and in the community, liaising with other health and social care professionals, to enable people to facilitate discharge from the community hospital to return to their homes in the community following illness or disability.
To provide high quality Occupational Therapy assessments where appropriate and provide direct rehabilitation where necessary managing patients with a range of conditions. This is an ideal opportunity for an Occupational Therapist wanting community experience to gain knowledge and leadership within an Integrated team at the forefront of new and exciting changes.
Responsibilities include:
1. Provide a rehabilitative approach to clients recovering from acute illness.
2. Conduct short term assessments into care facilities.
3. Manage longer-term conditions from an MDT.
4. Provide rehabilitative support for exacerbations of long term conditions.
5. Develop longer-term management plans (e.g., posture/seating and healthy lifestyles).
6. Signpost/referring to the wider community services.
You will need to be able to:
1. Triage and prioritize cases to effectively manage your caseload.
2. Display an organized and disciplined manner.
3. Work remotely and autonomously.
4. Be decisive and a team player, maintaining a good team rapport.
5. Supervise team members.
6. Display a passion for community rehabilitation.
Training and learning opportunities will be provided, and you will be supported within your post by a Team Lead for both formal and informal supervisions.
