Lead Best Interest Meetings to facilitate safe and timely hospital discharges in line with individuals' care plans. Collaborate closely with health board colleagues, therapists, families, and other external providers to deliver a seamless service centred around patient needs. Undertake Continuing Health Care Assessments, advocating for the best welfare strategies for the patient's discharge. Engage actively in multi-disciplinary team meetings to enhance patient outcomes and ensure alignment on discharge plans. Working from the Civic Centre in Neath, you'll be contributing to Adult Services within the Social Services Health & Housing structure, and be pivotal in shaping the future of hospital discharge practices.
