What you will do:
* Conduct assessments for adults being discharged from the hospital, ensuring their social care needs are met efficiently and effectively.
* Work collaboratively with health professionals and other agencies to create and implement personalised care plans.
* Empower individuals to make informed decisions about their care and support needs.
* Monitor and review care plans to ensure they remain appropriate and adjust them as necessary.
* Support the seamless transition of adults from hospital to home or community care settings.
* Maintain accurate and up-to-date records in line with statutory and organisational requirements.
* Utilise sound judgement and professional expertise to manage complex cases with confidence and skill.
