We have an exciting opportunity to be part of developing the delivery of the Kirklees and Calderdale Dynamic Support Register (DSR).
Your role will entail managing referrals, monitoring people on the Dynamic Support Register (DSR), and identifying people whose mental health concerns and/or challenging behaviour place them at risk of being admitted to hospital if they do not get the right care and treatment in the community.
This is a brand-new role created to ensure that locally we have a robust process that meets the national NHS requirements, ensuring that those supported by Learning Disability services have access to the DSR.
This role will work closely with several agencies to ensure that information on the register is live and communicated effectively to all stakeholders.

Main duties of the job
You will be a highly motivated and conscientious individual who has the desire to work creatively with people with learning disabilities within the community, providing the highest standards of safe and compassionate care to clients to maximise their potential.
You will use your excellent communication, co-ordination, and persuasion skills to achieve the best outcomes for the people we support.
You will coordinate Dynamic Support Registers (DSR) and chair meetings across both Kirklees and Calderdale, co-ordinating a multi-agency response.
You will also provide vital education and training in relation to the DSR across multiple agencies to raise awareness and best practice in line with the NHS DSR and CTR Policy (January 2023).
You will also provide information to meet national reporting requirements and in support of the Transforming Care Partnership Board and to ensure all requirements set out by the NHS DSR and CTR Policy (2023) are met and evidenced.

Job responsibilities
The post holder will co-ordinate the Dynamic Support Registers for Calderdale and Kirklees.
The aim of the DSR is to bring a person-centred and individualised approach to ensuring that the care and treatment and differing support needs of the person and their families are met, and that barriers to progress are challenged, overcome, and analysed, options compared and reviewed to agree the best way forward.
The DSR Co-ordinator would be responsible for bringing relevant professionals together to collectively discuss and agree a plan. The post holder is not responsible for making any care or clinical decisions but will gather relevant information into a central point.
The post holder will chair Kirklees and Calderdale Dynamic Support Register update meetings from which will populate/update information onto the Dynamic Support Register (DSR) from information provided by key workers across various agencies and provide data to relevant Departments/Agencies.
The post holder will be responsible for horizon scanning and keeping up to date with national guidance and embedding this into systems and processes to ensure a robust Dynamic Support Register to support the needs of the local population.
The post holder will provide education, planning, and deliver training around DSR processes supported by a standard operating procedure for learning disability and/or autism services across the Calderdale and Kirklees partnership.
Responsible for providing data and information to meet national reporting requirements and in support of the Transforming Care Partnership Board via a monthly DSR report which will also need to be linked with CTR delivery.
Person Specification
Qualifications
* Educated to degree level in a relevant subject related to health and/or care or able to demonstrate ability to work at that level.
* Postgraduate diploma in a relevant subject related to health and/or care or able to demonstrate ability to work at that level.
* Direct work experience of working within health care settings for people with autism and/or learning disability.
* Person Centred Planning with families and people with learning disabilities and/or autism.
Special Knowledge and Skills
* Significant experience/knowledge across a range of procedures, business case, computerised software, and project monitoring.
* Excellent and personable communication skills with a broad range of service providers and partner organisations.
* Ability to demonstrate a high level of skill and expertise in effective decision making.
* Ability to successfully analyse complex issues and situations and provide practical solutions.
* Knowledge of mental health legislation/law and other relevant policy and statutory guidance.
* Values differences and regards people as individuals.
* Ability to develop, establish, and maintain positive relationships with others both internal and external to the organisation.
* Able to work under pressure, dealing with peaks and troughs in workload.

Training
* Evidence of continued professional development relevant to health and/or care to the role.

Experience
* Significant knowledge and experience of working with people with learning disabilities and/or autism.
