An excellent opportunity has arisen within Hambleton and Richmondshire Urgent Community Response (UCR) and Hospital at Home Virtual Ward Service. We are looking for a skilled and outstanding nurse to join our UCR and Hospital at Home service within the community and growing the Friarage Collaborative.

The community nurse in UCR and Hospital at Home virtual ward will support the admission avoidance agenda.
Experience of working in a community setting, working within an emergency assessment unit, or rehabilitation would be an advantage, and we encourage informal visits to the areas.
The post holder will work as part of the multidisciplinary team as part of the Community UCR and Hospital at Home service to provide a comprehensive assessment and patient-centered care package, utilizing support services and liaising with other agencies to ensure the needs of this client group are met.
Main Duties of the Job
* Be an active member of the multidisciplinary team by using professional judgment and assuming responsibility and accountability for assessing, implementing, and evaluating clinical care for patients to meet their needs in a safe caring environment that promotes dignity, privacy, and respect.
* The post holder will be expected to develop knowledge and skills within the specialty and will assist in the management and organization of nursing work in the ward/department within the NMC Code of Professional Conduct.
* Actively support the admission avoidance agenda, providing high-quality care to people in their own homes, and work within a service that enables recognition of the early symptoms of disease, provides an appropriate response to exacerbation and sub-acute illness, and maintains a high-quality service to include palliative care and patients who need ongoing input within their own home.
* Work flexibly across 7 days a week, ensuring the 24-hour service is covered to meet the changing needs of the service, which includes working across the Hambleton and Richmondshire area.

After you complete this four and a half days of training, you will be able to explore further leadership and improvement training opportunities. We offer further in-house courses dependent upon your role and bitesize programs and leadership apprenticeships at levels 3, 5, and 7.

Knowledge

Qualifications
* Diploma or Degree in Adult Nursing.
* Registered as fit to practice by the Nursing and Midwifery Council (NMC).
* Evidence of Post Graduate clinical training/development.
Experience
* Commitment to evidence-based practice.
* Ability to work within the boundaries of the NMC Code of Professional Conduct.
* Experience of multi-disciplinary working.

Salary: £29,970 to £36,483 a year (pro rata)
