We are a small friendly team supported by 5 GP Partners. Our patient list size is around 8150 with a diverse population. We are a GP training and teaching practice and have a strong educational culture.
We are part of Lower Valley PCN and have many additional services such as physio, podiatry etc that is offered to all pts of the PCN.

Job responsibilities
Working in accordance with the UKCC Code of Conduct, the post-holder will work within the practice nursing team in the provision of assessment, treatment, screening, health education services and advice to patients.
The post-holder will work with reference to practice agreed protocols and guidelines to provide nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
The post-holder will have the required level of training and competence and will work within patient group directives where these are available.
Support the Practice Infection control lead in policy development; implementation of standards and provide specialist infection control advice to all staff and clinicians.
Clinical Responsibilities:
Identify, assess and diagnose patients with complex health needs across the practice population, in all cases in accordance with practice policy & protocols
1. Support the use of technical and specialist nursing treatments for patients with complex healthcare needs
Assess and advise patients by phone, including patients who self refer with minor illnesses.
2. Be involved in developing programmes to improve health and well-being and meet the specific needs of communities, groups and individuals
3. Work with others to change policies to improve health and well-being and reduce inequalities
4. Work with colleagues in the implementation and evaluation of evidence-based practice to improve the services offered by the practice nursing team
5. Work within the team in quality assurance, development and clinical governance.
6. Work with other members of the practice team in the implementation of specific aspects of the NSFs.
Management of Chronic Disease:
7. Nurse Led Chronic Disease Management
8. Identifying significant abnormalities
9. Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
Therapeutic Monitoring:
10. Checking compliance with and adherence to appropriate treatments using an holistic patient-centred approach
11. Recognising abnormalities
12. Identifying the impact of treatment and implementing or recommending changes as appropriate.
Ear Care:
13. Providing routine ear care
14. Dealing with more complex problems and prescribing if appropriate
Health Screening:
Recognising issues and gathering sufficient information to refer ( womens and mens health, sexual health, older people)
Undertaking monitoring tasks ( smears) and providing advice as appropriate
Providing in depth monitoring and advice as appropriate and working with the patient in deciding on management plans
Considering the issues in light of the practice population.
Immunisation (Adult and Child):
15. Administering appropriate immunisation autonomously
16. Working with patients with more complex immunisation issues.
17. Working with public health initiatives ensuring the work of the team is up to date.
18. Leading the team in achieving the immunisation guidelines and targets
Mental Health:
Identifying problems and providing guidance in accordance with guidelines.
Ensuring that appropriate systems are in place for effective patient monitoring.
Minor Surgical Procedures:
19. Provide nursing support in the delivery of the service as required
Patient Health Checks:
20. Working with patients to develop a management plan obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues
21. Identifying health trends and public health issues and working with the team and others to combat these.
Risk Assessment:
Recognising issues and gathering sufficient information to refer (eg drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems)
Travel Health:
22. Administering injections and providing guidance in accordance with guidelines

25. Working with public health initiatives ensuring the work of the team is up to date.
Wound Care and Management:
26. Assessing routine wounds including trauma

Support Practice Infection Control Lead
Work with the practice lead promoting high infection prevention & control (IPC) standards in clinical treatment and throughout the practice to ensure a quality service.
Assist in compilation and implementation of policy, guidance and procedures relating to IPC in the practice
Assist in the collection and contribute to the analysis of surveillance / audit data
To identify, investigate and monitor infections and associated hazardous practice / procedures which could pose health risks to patients, staff or the public, reporting findings and actions required to the Practice Manager
To provide specialist advice to staff, patients and the public, in all matters relating to IPC to improve on patient/client care and to develop others.
To communicate as required, with patient/relatives/healthcare workers/outside agencies and give advice/reassurance relating to Infection IPC or the implications of microbiological results.
To maintain updated knowledge and skills in IPC and feeding back / share information appropriately on matters relating to IPC to all practice staff.
