We exist for our local population, now and into the future, and we believe that we can achieve more together by working in partnership with others Description of Service Area Rehabilitation and Enablement The Rehabilitation Team work with individuals on the inpatient unit, attending as outpatients (either individually or in a group), via virtual sessions, and sometimes in the community. We aim to enable the individual to achieve what matters to them by optimising strength, fitness and balance, and/or by modifying and adapting their approach. We provide educational group sessions to inform and empower people living with a life-limiting condition and associated symptoms. Self-management advice and strategies can enhance the person’s ability to live well with their condition and gain greater quality with less dependence on health professionals and medication. Holistic Care End of Life Care takes a holistic approach, focusing on the whole person and involves the patient and their family/care givers. Holistic care includes physical, psychological, emotional, social, and spiritual care. Multi-disciplinary Team Consultants in palliative care; medical doctors; nurses; healthcare assistants; physiotherapists; occupational therapists; clinical psychologists; art/music/drama therapists; counsellors; social workers; pharmacists; spiritual care staff.
