Job Summary
This post is fixed term for 36 weeks to cover maternity leave.
As part of the Healthy Child Wales Programme, the Specialist Community Public Health Nurse - Health Visiting (SCPHN - HV) will be the named Health Visitor for all Children and their families within their caseload from antenatal to 5 years, excluding the unborn child. This requires the Health Visitor (HV) to commence the child's planning process.
To work without direct supervision to undertake statutory duties on behalf of the Health Board with respect to individual children within your caseload to assess, monitor, improve and support the wellbeing of children and their families.
Working in partnership with the child's family and all involved agencies to produce a child's plan which will require to be monitored, reviewed and evaluated.
In discharging these responsibilities the post holder is responsible and accountable for maintaining clinical, staff and information governance.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
1. Be responsible for providing a Health Visiting service for a defined caseload of families with children aged 0-5 including in the antenatal period. Ensure transfer to the appropriate service at appropriate time.
2. Provide a child centred, inclusive, holistic and accessible Public Health programme to children, their families and communities.
3. As part of Healthy Child Wales Programme (HCWP), quality assurance framework for current practice, skills such as motivational interviewing are required.
4. As appropriate, lead and/or contribute to developing strategies to address individual identified needs as well as those identified within the community.
5. Undertake an assessment using the recognised and accepted acuity tools, to support safe staffing levels and support colleagues during fluctuations in workforce and workload activity.
6. Participate and chair Health Visiting Forums, team meetings and working groups as required, and participate in projects regarding future service development, developing and implementing identified changes to the routine and delivery of routine care.
7. Lead and deliver the Universal Healthy Child Wales Programme (Welsh Government 2015) which is underpinned by the Quality Assurance Framework being adaptable and flexible when models of care change.
