This includes providing emotional, social, and practical support to complexly poorly children and their families, as well as bereavement support to families in both group and 1:1 settings.

Main Purpose of Job
To design, develop, and deliver a diverse range of safe, innovative, recreational, and therapeutic activities for CYP. This includes offering emotional, social, and practical support to children with complex medical conditions and their families, and bereavement support to families through group and 1:1 interventions. You will also help evaluate and improve the Championing CYP Project, ensuring funders' outcomes are met.
Key Responsibilities
Design a support program for CYP aged 0-25, co-created with them to ensure their voices are heard and the activities inspire and empower them.
Conduct risk assessments to ensure activities are delivered safely.
Provide regular activities to reduce social isolation for the CYP we support.
Monitor service delivery and provide regular updates to the CEO and Trustees.
Foster relationships with key partner organizations.
Collect evaluation data to measure the project's impact and success.
Ensure targets and outcomes are met in line with strategy and funder requirements.
Stay updated with policy changes affecting CYP support services.
Provide clear written communication and be competent with Microsoft Office.
Plan and administer all work to a high standard.
Working with Beneficiaries and Service Delivery
Respond promptly to all support requests, maintaining a heart-centred approach.
Conduct initial needs assessments for CYP and design individual support plans with families.
Work with the CEO and Family Team to review and monitor CYP support and plan activities.
Host events and source qualified staff and volunteers.
Attend professional meetings and maintain effective relationships with health professionals.
Collaborate with charities and organizations to raise awareness and appropriately signpost services.
Ensure confidentiality and data protection at all times.
Work flexibly across various venues, with evening and weekend work required.
Person Specification
Experience
Proven experience designing, leading, and delivering safe, recreational, therapeutic, and inclusive activities for CYP (E).
Experience working with CYP in group settings and 1:1 (E).
Experience developing creative support programs in partnership with CYP (E).
Experience working with bereaved CYP (D).
Experience with children and families with complex needs (E).
Knowledge of safeguarding practices and procedures (E).
Experience in project evaluation (D).
Skills/Knowledge
Relevant qualification in health, social care, childcare, or youth/community work at Level 5 or equivalent (E).
Strong communication skills to engage CYP from diverse backgrounds (E).
Excellent verbal and written communication (E).
Understanding of social isolation reduction interventions for CYP (D).
Ability to prioritize, work independently, and take initiative (E).
Strong organizational, planning, and time-management skills (E).
Proficient in Microsoft Office and related programs (E).
Competence in social media use and management (E).
Full driver’s license and own car, with regional and occasional travel (D).
