A new exciting opportunity has arisen for an experienced clinical manager to lead the implementation of personalised stratified follow-up and remote monitoring for cancer patients deemed eligible for self-management, supporting individuals through their follow-up pathway, ensuring a safe effective service.
The post holder will be required to operationally lead a small team responsible for the digital management of Personalised Stratified Follow-up (PSFU) using a digital remote monitoring solution.
The post holder will ensure that patients referred to Remote Monitoring follow the correct pathway, taking responsibility for coordinating activity schedules including blood tests/imaging and reporting outcomes to the patient, their GP, and clinical teams.
We would welcome applications from individuals educated to degree level with evidence of continual role development. Interested applicants must have a minimum of three years’ experience in an administrative role, extensive IT skills, and in-depth knowledge of the Somerset Cancer Registry, along with a proven track record of monitoring healthcare targets. Knowledge of the national personalised care agenda is essential, along with experience of multi-professional working and managing other staff.
Good communication, interpersonal, and organisational skills are a key component of the role, along with the ability to analyze data and produce reports.
