Job summary Fixed Term Contract - 12 months This is an exciting opportunity for an experienced community nurse to work within a specialist Homeless and Vulnerable Adults Nursing Service on behalf of Bolton NHS Foundation Trust. The role involves outreach work to reach those experiencing homelessness, including those in temporary and hostel accommodation. The team also provide regular drop in clinics in a central location. The team also deliver a valuable in-reach to those patients experiencing homelessness in a hospital setting in order to advocate and safely maintain patients within the hospital and also provide a safe discharge into temporary accommodation. The role will involve working closely with Bolton Council's Rough Sleeper Outreach Team and Rough Sleeper Drug and Alcohol Team. Local charities including Homeless Aid UK and Urban Outreach and other health services including North Manchester Blood Bourne Virus team, Diabetic specialist Service, Adult Safeguarding and Achieve. This role needs a compassionate, caring nurse with a non-judgemental approach towards caring for some of the most vulnerable patients in Bolton. Main duties of the job We are seeking enthusiastic team players, to work within a dynamic multi-disciplinary health team, with excellent communication and interpersonal skills, and the ability and experience to manage complex cases in partnership with other agencies. A keen interest in working in the homeless sector is required however we understand that not everyone will have worked in this sector so experience is not essential.
