About the Role: As a Social Worker in our Mental Health Team, you will play a vital role in supporting individuals with mental health needs. Your responsibilities will include: Managing a complex caseload of individuals with a range of mental health conditions. Conducting comprehensive assessments, including mental health assessments, and developing personalised support plans. Working collaboratively with service users, their families, and carers, promoting self-advocacy and empowerment. Liaising and collaborating with a range of partner agencies, including mental health services, community health teams, housing providers, and voluntary organisations. Providing crisis intervention and support as needed. Safeguarding vulnerable adults and adhering to all relevant legislation and policies. Maintaining accurate and up-to-date records. About You: Qualified Social Worker registered with Social Work England. Demonstrable experience of working with individuals with mental health needs. Sound knowledge of relevant legislation and guidance, including the Mental Health Act 1983 and the Care Act 2014. Excellent assessment, analytical, and communication skills. Ability to work independently and as part of a multi-disciplinary team. A commitment to promoting recovery and wellbeing.
