Your role:
We require a social worker to join our busy community front door team at Wigan Council. This is a fast-paced role within adult social care working under the Adult Front Door model of practice, working within an asset based approach.
This would include the following:
* Conducting assessments to identify risks for vulnerable adults.
* Developing and implementing care plans tailored to individual needs, in collaboration with residents and their support networks.
* Conducting home visits and telephone assessments and contact with residents to monitor their safety and well-being.
* Coordinating with external agencies, healthcare professionals, and community resources to provide holistic support to residents.
* Participating in multidisciplinary meetings to review resident progress and develop intervention strategies.
* Providing crisis intervention and support to residents experiencing immediate safety concerns.
* Maintaining accurate and up-to-date records of resident interactions, assessments, and interventions in accordance with regulatory standards.
About you:
We’re looking for someone who has significant social work experience and crisis intervention within Adult Social Care.
You will have a degree or equivalent in Social Work and will be registered with Social Work England. You will be driven and confident at working in a fast-paced environment and have the ability to contribute to the effective running of MDT discussions with excellent communication and organisational skills.

The multi-disciplinary team consists broadly of Occupational Therapy, Social Care, Reablement and Assistive technology staff, as well as Customer Service Advisors and Coordinators who handle calls/screen and prioritise referrals. The team aims to respond to urgent need and to maximise opportunities for independence, prevention services, information advice and signposting. This is an exciting opportunity to play a key role working as part of a multi-disciplinary team, promoting a joined-up approach within the team and with wider partners to achieve the best outcomes for our residents.
The team is essentially office based with some flexibility.
