Job summary
The post holder is anexperienced nurse who, acting within their professional boundaries, will providecare for the presenting patient from initial historytaking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinicaldecision-making and expert care, including assessment and diagnostic skills,for patients within the general practice. The post holderwill demonstrate criticalthinking in the clinical decision-making process. They will work collaboratively with the generalpractice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurseleadership as required. In order to work at this level NMC requirements foradvanced practice must be met.
Main duties of the job
They should have experience in management of minor illness, minor ailments and injuries and chronic disease management. They should be able to participate in patient telephone triage to manage patient demand and advise patients in the safest and most appropriate way relevant to their problems. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. Will undertake home visits.

You will join a clinical team consisting of 2 Partners, 5 salaried GPs, 1 Advanced Nurse Practitioner, 5 Practice Nurses, 4 HCAs, 1 Paramedic and 1 Clinical Pharmacist.
Sidley Medical Practice has been the host site for the COVID vaccination program, working closely with the Bexhill Primary Care Network and other local GP surgeries.

Clinical practice:
Assess, diagnose,plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
Clinically examineand assess patientneeds from a physiological and psychological perspective, and plan clinical care accordingly
Assess, diagnose,plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose andmanage treatment plans for patients at risk of developing a long-term condition(as appropriate)
Diagnose and manage both acute and chronic conditions, integrating both drug-and non-drug-based treatmentmethods into a management plan
Prescribe andreview medication for therapeutic effectiveness, appropriate to patient needsand in accordance with evidence-based practice and nationaland practice protocols, and within scope of practice
Work with patients in order to support compliance with and adherenceto prescribed treatments. Provide information and adviceon prescribed or over-the-counter medication on medication regimens, side-effects and interactions
Prioritise healthproblems and intervene appropriately to assist the patientin complex, urgentor emergency situations,including initiation of effective emergencycare
Support patientsto adopt healthpromotion strategies that promote healthylifestyles, and apply principlesof self-care
Communication:
Utilise and demonstrate sensitive communication styles, to ensure patientsare fully informedand consent to treatment
Communicate with and supportpatients who are receiving bad news
Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcomedifferent levels of understanding, culturalbackground and preferredways of communicating
Anticipate barriers tocommunication and take action to improve communication
