Key Responsibilities
* Conduct comprehensive assessments to develop individualised intervention plans for stroke patients.
* Deliver therapeutic interventions to promote recovery, independence, and functional abilities.
* Work collaboratively within a multidisciplinary team, ensuring seamless and effective patient care.
* Provide education and support to patients, families, and caregivers to enhance rehabilitation outcomes.
* Maintain accurate records of patient progress and treatment plans, ensuring compliance with professional standards.
* Valid HCPC registration.
* Experience in stroke care (highly desirable).
* Valid UK driving licence to facilitate travel between various healthcare sites and community locations.
