Responsibilities
Conduct comprehensive assessments of clients' needs and develop individualized care plans.
Provide ongoing support and counseling to clients and their families to navigate social services.
Collaborate with healthcare professionals, community organizations, and legal entities to coordinate care.
Advocate for clients' rights and access to necessary resources across various organizations.
Monitor and evaluate client progress, adjusting care plans as needed to meet changing needs.
Educate clients and families about available services and their benefits.
Document client interactions, progress, and interventions accurately and timely for compliance and reporting purposes.
Requirements
Bachelor's degree in Social Work (BSW) or equivalent experience in a related field.
Current state licensure or certification as a Social Worker.
Experience working with adults in a social work capacity or similar role.
Strong understanding of social, economic, and environmental factors affecting client well-being.
Excellent communication and interpersonal skills to build rapport with clients and families.
Ability to work independently, manage time effectively, and make decisions in high-pressure situations.
Commitment to ongoing professional development and adherence to ethical standards.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £20.36
Location: Hereford, Herefordshire
