Job overview
We have a great opportunity for a full-time band 6 nurse to work in Intensive Support Element of the Dementia & Older People's Mental Health Team based in Haywards Heath West Sussex. Intensive Support operates a to 9-5pm 7 days a week service including Bank Holidays.
The successful candidate will join a friendly, highly-skilled and enthusiastic multi professional team. We will take a keen interest in helping you to develop the skills important to this role and the post-holder will be encouraged to develop special interests and expertise in line with service and team objectives.
You will be part of a highly-skilled and supportive multi-disciplinary team. You will receive regular managerial and clinical supervision. This is challenging but rewarding work as we regularly receive positive feedback from our service users and carers. Sussex partnership NHS Foundation Trust has recently achieved outstanding for the care it provides.
The post holder will be expected to travel across the geographical areas of Mid Sussex covered by the team, the ability to travel to remote areas is essential.
Main duties of the job
The role of this post is to actively engage those requiring an increased level of support with the aim of keeping people within their home and maintaining their wellbeing and independence as far as possible. We are looking for individuals who are dynamic, compassionate, good communicators who are able to demonstrate a holistic and person-centred approach to their work.
You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach and within that context you will manage a caseload of older adults.
You will need to be caring, compassionate and a good communicator, with a commitment to improving the lives of our service users through creative, evidence based practice. You will be responsible for working collaboratively with service users and their carers to create care plans that meet their needs and maximise their independence.

Job responsibilities
Clinical
* To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.
* To support/manage a team caseload and attend relevant MDT meetings.
* To regularly support and cover the duty rota.
* To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway.
* To provide risk screening and assessment as well as risk management planning.
* To contribute to and facilitate evidence-based assessments and individual and group interventions.
* To contribute to the maintenance and development of the Service.
* To regularly share and supervise students on practice placement, and junior staff as appropriate.

Essential criteria
* Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role.
* Completion of Mentorship Course/ENB equivalent.
* Evidence of continuous professional development.
Knowledge/Experience
Essential criteria
* Significant post qualifying experience in the relevant care group.
* Experience of working alongside people with complex health problems in the designated care group.
* Experience of supervising staff.
* Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems.
* Experience of working consultatively with professionals.
* Experience of conducting clinical assessments including risk assessments.
* Experience of partnership working within and across various statutory and non statutory teams and agencies.
* Experience of working within the CPA process.
* Understanding of clinical governance.
* Previous experience of working in the community.
