We have an exciting permanent opportunity to join our Mental Health Social Work Team as a Social Work Assistant. As part of this role, you will be responsible for the assessment of individuals' care and support needs and the reviewing of packages of care in line with the East Renfrewshire HSCP Supporting People Framework. You will provide support to adults with mental health needs and their families/carers who may require advice and guidance. You will also provide practical assistance where necessary. You will contribute to facilitating safe, person-centred discharge from hospital for individuals subject to mental health legislation, working closely with Mental Health Officers and Social Workers within the team. You will also work closely with relevant professionals within the Adult Community Mental Health Team to ensure a holistic approach to the care and treatment of service users and with partner agencies such as health, housing, and the third sector to achieve positive outcomes for individuals.
We are looking for someone who has experience of working within a health or social care setting supporting individuals to live well within their community. The applicant should have knowledge of mental health and recovery and how this applies to working within a strength-based ethos.
Ideally, the successful post holder will also have a Social Care or Health qualification as well as experience of using an electronic recording system.
