About the Role
We are seeking a dedicated and compassionate Social Worker to join the Chesterfield Royal Hospital Discharge Team within Adult Social Care. This essential role supports individuals during their hospital stay, promoting independence and enabling safe, timely discharges to avoid unnecessary long-term hospital admissions.
You will work within a fast-paced Integrated Multi-Disciplinary Team (IMDT), providing person-centred, strengths-based assessments and short-term support planning. This is an exciting opportunity for a proactive and resilient social work professional who thrives on delivering high-quality care in a collaborative environment.
Key Responsibilities:
Undertake assessments and care planning in accordance with the Care Act 2014, focusing on promoting independence and personal strengths.
Facilitate safe, effective, and timely hospital discharges through partnership working.
Liaise effectively with individuals, families, carers, health professionals, and other agencies.
Apply the principles of Making Safeguarding Personal and work to safeguard adults in line with statutory guidance.
Carry out Mental Capacity Act assessments and support best-interest decision-making.
Accurately record and manage casework to reflect the quality and standards of the service.
Essential Qualifications and Requirements
Social Work degree (BA, BSc, MA, or equivalent) or Postgraduate Diploma in Social Work.
Registration with Social Work England (must be maintained throughout employment).
Sound working knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding adults procedures.
Experience in adult social care settings, preferably including hospital discharge or multi-agency working.
Commitment to person-centred and strengths-based approaches to practice.
Excellent communication, interpersonal, organisational, and problem-solving skills.
The ability to work under pressure and manage competing priorities in a fast-paced environment.
Desirable
Previous experience within a hospital or urgent care setting.
Knowledge of community services and resources available to support hospital discharges.
Experience with electronic case management systems.
