Salary: £46,148 - £52,809 Per annum

Job Overview:
We are looking for a passionate clinician to join our service and lead our Intensive Health Outreach Team (IHOT). You will be responsible for leading a team that strengthens the NHS contributions to prevention and tackling health inequalities, ensuring excellent standards are upheld within the team.
Key Responsibilities:
1. Lead in the operational and clinical management of the Service, providing day-to-day management and leadership to staff.
2. Undertake specialist assessment, planning, implementation, and evaluation of care in the community for service users with severe/complex learning disabilities.
3. Demonstrate enhanced competence in complex decision-making and the management of clinical needs.
4. Collect and collate data for audit, research, and service performance.
5. Work in partnership with service users, carers, and other agencies to ensure effectiveness of outcomes.
6. Communicate complex and sensitive information to service users, family members, and other clinicians.
Qualifications and Experience:
* Degree and active registration in Learning Disability, Mental Health or Adult Nursing or equivalent core Health role.
* Comprehensive experience of working with complex physical health needs for people with learning disabilities.
* Experience in Team Management.
